Most of the procedures we do require a report. Usually the definition of the cpt code includes that. I don't see that requirement in the code definition of the ILR implants.
The hospital scans the device company's ILR registration form in where the op report would go. It lists off hospital name, implant doctor name and address, patient demos, indication and various related model and serial numbers.
For most procedures this would cover the device billing not the professional services billing but since the cpt doesn't ask for a procedure report or description, will this suffice as documentation to bill the doctors' portion of the ILR implant or should the doctor dictate a report?

No, For Professional services, i.e., implanting of ILR implants, Doctor needs to dictate the operative report.
