Would it be appropriate to use 64999 for iO-Flex Baxano device being used during a laminectomy, foraminotomy & facetectomy procedure for foraminal stenosis? The description of the device is to create a wider foraminal opening and our surgeon's operative report states the same.

iO-Flex Baxano device is a microblade shaver flexible instruments which is used for lumbar decompression and will not be coded separately as it is inclusive in laminectomy, foraminotomy & facetectomy procedure.
