Would the correct coding be:
36831 (Thrombectomy), 36147 (Fistulagram), 37197 (Stent Removal), 35903 (Graft Removal)

1. CPT code 36831 has been billed for thrombectomy. This is the code for open thrombectomy. In case, the procedure performed is percutaneous thrombectomy, the appropriate code would be 36870.
2. CPT code 37197 is the correct code for removal of intravascular stent.
3. CPT code 35903 has been billed for removal of the AV graft but the location has not been mentioned. This code is location based and there are multiple codes for different locations. CPT code 35903 is for extremity, code 35901 is for neck region, code 35905 is for thorax region and code 35907 is for graft procedure performed on abdomen.
4. CPT code 36147 is the correct CPT code for percutaneous AV fistulogram.
