We have been trying to determine the best code to use for intraperitoneal drain placement for free air. The provider suggested code 49020 but that does not seem to be correct. In the past there was a code 49420 Insertion of intraperitoneal cannula or catheter for drainage or dialysis; temporary which seems like it could fit, but the new codes that are suggested after 2011 do not seem to fit. Please see the description of the procedure below and see if you can help point us in the correct direction.
A stab incision was created in the right lower quadrant through his old drain site. A moderate amount of murky fluid and a small amount of air was evacuated. A quarter inch Penrose drain was cut to size and then inserted into the abdomen without any difficulty and was then sutured in place to the skin using a 3-0 prolene. It was then cleaned, dried and dressed in gauze and Tegaderm.

Code 49020 is correct for this scenario. In this procedure, the provider incises the abdomen and drains an area of infection or abscess from the abdomen. Provider may insert a drainage tube too.
