Presumably this is a dialysis shunt. Important considerations include learning to distinguish between venous and arterial services and understanding how CPT defines segments. As a place to start:

For the fistulogram review the definition and guidelines for 36147.

For the angioplasty review the definition and guidelines for 35476, 75978

For the stent review the definitions and guidelines for 37205, +37206, 75960
