Interspinous injection

Eunice Posted Wed 20th of November, 2013 09:24:45 AM

I am hoping someone can advise if I have the correct CPT code for an "L4-5 interspinous injection of marcaine and steroid with fluoroscopically guidance."
I am thinking CPT 20550 inj, single ligament, therapeutic., with 77003.
Thank you!

Detail documentation would help me reach the correct code.

The patient was given 10 mg of Valium orally. The patient was placed prone on the table. After infiltrative anesthesia with 1 percent lidocaine, the 22-gauge spinal needle was also placed in to the L4-L5 interspinous interval. The placement of the needles was confirmed utilizing 4 ml of Isovue 200M. Subsequently, Marcaine and Kenalog was injected.

Thank you!

Still there is no mention of ligament in your OP report. It is always better to clarify with the physician in such cases.

Most of the times it is ligament only but 'not documented not done' is the golden rule of Coding.

If in Your OP report needle is inserted in the CL4=L5 ligament, hence 20550 Injection(s); single tendon sheath, or ligament, aponeurosis (eg, plantar "fascia")qualifies much better than any rest of the codes.

Codes can vary depending upon the various scenario, its always better to ask for complete information to properly judge the code sets.
