Interpretation and Report

Many codes in the medicine section for Opthalmology require interpretation and report. What I am not finding in specific documentation like CMS/CPT as they are very vague, what "report" means. Does report mean that a separate sheet/form is required for the "report".
Can you explain or provide links to documentation that shows what "interpretation and report" means and how much and what information is required? An opthalmology provider simply documents one or two words in his interepetation section, which I belief is too brief, but no report is attached. They own the equipment and billing for TC/PC. Thanks
