Need help on "J" codes for Novolog 40u and Lantus 40u.

NovoLog is indicated for the treatment of patients with diabetes mellitus, for the control of hyperglycemia. NovoLog may be infused subcutaneously by external insulin pumps. NovoLog may also be administered intravenously under proper medical supervision in a clinical setting for glycemic control.

J1815 represents 5 units of insulin. For 40 units, J1815 with 8 units.
But if Insulin pump is used as usually, HCPCS Level II code J1817 represents 50 units of insulin administered by DME through an insulin pump.

Still not clear about coding for Lantus. Will be coming soon with an answer.
