How would you code insertion of a vagal nerve stimulator and of vagal nerve electrode array with interrogation of the stimulator programmer?

You should report 64568 (this includes both nerve stimulator and electrode placement)with 95974 for interrogation for one hour duration, if interrogation go beyond an hour then add 95975 for that additional time.

Is the programming include in the service of neurostimulator implantation, revision, and removal and should only be coded at a later time than the implantation? Please list your resources.

Yes, the provider makes an incision and implants a neurostimulator electrode array on the vagus nerve and one of the cranial nerves, and also implants a programmable pulse generator. Hope it helps!

This the protocol to program the device while placing it. So, it cannot be billed separately.

Cpt code 95974 has in the description of the code "with intraoperative or subsequent programming," so wouldn't intraoperative mean, "occurring during a surgical operation." Your thoughts?

Well, section specific guidelines of CPT code 95974 indicate that CPT code 64568 should be used when neurostimulator pulse generator inserted. So, you can bill both the CPT code 64568 and 95974 together. CCI edits do not show any bundling in between these 2 codes.
