Hi, I need to code a patient that was admitted to the hospital for PIH, insulin dependent diabestes mellitus and the flu for 2 days before the delivery. Patient is 35 weeks. Had complications with PIH and Polyhydramniosis,non reassuring fetal heart tones. On the 3rd day the patient had a C-section and tubal ligation. The patient did not go home before the c section. can I bill 2 day hospital admission 99221 and 99231 then bill the global 59515 and 58611 at the same time. If so do I need to add any modifies for the cpt's Thank you Cathy

Codes 59515 and 58611 should be appropriate. It is also suggested to ask the payer preference when going ahead for the final billing. Thank you.

Hi. but my questions was not answered? In patient hospitalization during the pregnancy is not considered part of the global is it?

We can bill hospital admission codes 99221 and 99231 with modifier 24 with global OB period. Reason being If a patient develops complications of pregnancy or the provider treats the patient for an unrelated problem, these visits are excluded from the maternity global package and can be reported separately. Append modifier 24 Unrelated evaluation and management service by the same physician during the global period to all E/M services that address the pregnancy complications or unrelated issues. Modifier 24 is needed to alert the payer that the E/M service(s) is unrelated to the global OB package. Thank you.
