Hello,
The below procedures...is fluoroscopic guidance / epidurography included or are we able to bill out as a separate procedure?

We would use 77003 or 72275 with this code range:

Would we use 77002 or 77003 with the below codes?

Ans- You just need to check CCI edit code 77002 or 77003 with codes mentioned by you. If CCI edit says that a fluorosocopy or needle localization is included in other code but a modifier is allowed to bill them together, then you may bill them using modifier 59, distinct procedural service. Make sure that your documentation support a distinct service. Typically you may not bill fluoroscopy or needle localization when the descriptor of other code says that code include in them fluoroscopy or needle localization. See for phrases such as "including fluoroscopic guidance", "including needle localization".

Question- We would use 77003 or 72275 with this code range: 62310 62311 -
Ans- There is no edit of code 77003 with 62310-62311. You may bill them together.

As per CCI edit, 72275 is included in 62310. You may bill them together by appending modifier 59 with 72275. Make sure that medical records include a separate report for epidurogram to qualify 72275 as a distinct procedure. Thanks !!
