I work in an ASC & have a question in regards to coding bilateral injections.
For example:::Pt is seen of a median branch block (lumbar) bilateral L4, L5, S1--Would you code it as 64493-50, 64494-50 or would it be 64493-50, 64494-50, 64495-50?

It would be 64493-50, 64494-50. If there is any additional level, then you can bill along with these codes 64495-50, but remember you can not report 64495 more than once per day. It would be better if you will elaborate the description with levels.
