We were doing an infusion on a patient and the patient had an adverse reaction to the medication. We normally bill CPT code 96365 and J3358. The infusion lasted 33 minutes. J3358 was stopped due to pt c/o chest pain, S.O.Br, facial flushing, approx. 20mls infused. and 32 mls were wasted. Or 200mg were used and 320mg's were wasted. How would this be billed?

Hello Susan,

Thank you for your question.

As per the above documentation, report CPT® code 96365 for intravenous infusion for 33 minutes.

For drug code J3358, report the J-codes as below.

J3358 x 200 units

J3358 x 320 units -JW

Note:  The JW modifier is to report the amount of drug or biological (hereafter referred to as drug) that is discarded and eligible for payment under the discarded drug policy. The modifier shall only be used for drugs in single dose or single use packaging.

Hope that helps.
