Immunizations and V03-V06 diagnosis codes

We have a question regarding immunizations and their specific V codes. A patient comes in for a HPV vaccine. The patient is counseled by the doctor and it is administered by the nurse. Is the correct billing 90649 and 90460 with diagnosis code V04.89 or should we bill 90649 and 90460 with diagnosis code V05.9? Would we need to bill the specific diagnosis code for the vaccine?

Thank you for your response .. So, for all vaccines, they should be billed with their specific V code (V03-V06) if the patient comes in for just the vaccines? When a patient comes in for their well/routine visits we would bill the vaccines underV20.2` diagnosis .. is that correct?

Yes. When a well child visit occurs, the primary diagnosis code is V20.2.
