Surveillance only following completed treatment involves V67 category codes. So, if the wound is healed, then you should use V67 category codes. You need to pick best possible options as appropriate:
V67.00 Following surgery, unspecified
V67.09 Following other surgery
V67.59 Following other treatment
V67.6 Following combined treatment
V67.9 Unspecified follow-up examination
ischemic of toes on r foot:
The detailed condition with documentation is not there. Yet, I can assume that if the ischemic toes or foot is(as is usually) a result of atherosclerosis of native extremity arteries, then the code should be 440.24
