physician only noted: Chemotherapy induced neuropathy and diabetic neuropathy.

Would the codes be E11.42 and G62.0 but don't know what code to attach would it be T45.1x5?

The set of series of ICd 10 codes which you have given seems to be appropriate. As per the guidelines with G62.0 we can assign the ICD 10 codes from T36-T45.
