Impression reads: "non healing post traumatic R pretibial wound. Patint has mixed venous insufficiency and peripheral arterial disease."

I coded this patient:
459.81 - Venous Insufficiency
707.19 - Lower Limb Ulcer NEC
443.9 - Peripheral Vascular Disease NOS

Is this accurate?

It depends on the type of report and the "treatment direction" in particular. For example, if the focus of treatment in this case is "non healing post traumatic R pretibial wound", then code for this would be Primary. So, the basic thing first you need to decide from the report is the "Treatment Direction".
