ICD generator replacement

Dx: Z45.02
The patient presents for replacement of his DDD-ICD generator.
The existing RV defibrillator lead is not functioning well, so it is capped.
A new RV defibrillator lead is inserted.
The old DDD-ICD pulse generator is removed and a new DDD-ICD generator is inserted, and connected to the new RV lead and existing RA lead.
Defibrillation threshold not done.

CPT: 33249, 33241

Is the billing correct?
Can you combine 33249 and 33241?
Any modifier?

CPT-33249,33241 seems appropriate.
