What ICD-9 code can I use for junction breakdown at L4-L5??

Junction breakdown can be either a fracture, arthropathy, osteoporosis or anything. So need to provide some more detailed information.

I know, but I bill for an ASC and all I have is the operative report, that's it! so I was wondering if maybe there was an unspecified/NEC icd-9 code I can use instead? the only other thing the report mentions is lumbar radiculopathy & lumbar disc herniation 722.10 & 724.4 so that's what Im using.
