ICD-9 codes

Is it mandatory for ICD-9 codes to be displayed in the Assessment and Plan section of a progress note or just a description of a condition is sufficient enough for legal/audit purposes? Lets say if patient is being diagnosed with Acne, does 706.1 have to appear next to this diagnosis in the note?

I am not aware of any payer requirements regarding mandatory documentation of the actual ICD-9 code(s) versus clear documentation of the patient's condition / diagnoses. In addition the ICD-9-CM Guidelines also do not include any such requirement.
