My doc performed a exploratory lap with lysis of pelvic adhesions, ureterolysi, resecton of adenexal mass and hydrosalpinx.

The most appropriate code according to the above mentioned scenario is CPT 58662, 50949-59. ICD-10 for Hydrosalpinx is N70.11.
The code 50949 would be appropriate for the laparoscopic ureterolysis since there is no specific code for this procedure.
Add the modifier 59 (Distinct Procedural Service Under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-E/M services performed on the same day.
