HX of polyps vs Screening

One of our patient came in for a colonoscopy after 5 years. We coded Z86.010. She called and said that her insurance pays for a screening (Z12.11) every 5 years. Would is be right to correct the coding to Z12.11 for primary and Z86.010 to secondary?

It is appropriate to code Z12.11 as primary and Z86.010 as secondary. As the patient is seen for screening.

I have a follow up question. Would you code G0105 for for hi risk or G0121?

The appropriate code for high risk patient will be G0105 as the code states Colorectal cancer screening; colonoscopy on individual at high risk.
