The patient was taken to the operating room today for exploration of his transplant ureter, ureteral lysis, excision of distal ureteral segment, and complex ureteral re-anastomosis to the bladder, with stent placement. Because of the extremely low position of the kidney, the ureter coming around the vessels was involved in periosteal scar, and then acutely angulated upwards on the anterior surface of the bladder, extending cephalad for length of at least 5 cm.
