J3370-Injection, vancomycin hcl, 500 mg
J1100-Injection, dexamethasone sodium phosphate, 1mg
J0713-Injection, ceftazidime, per 500 mg

If the vials are single dose vials, you would need to code the above with one unit and document the waste with modifier JW, one unit. Both codes are processed for payment so you would reimbursed for the wasted solution.
