How do I code shaved lesions in the same body area?

I have 4 lesions shaved off the neck area, and they were all .5cm or less. (cpt code 11305) It's being billed to United Healthcare, and I was thinking I would bill 11305, 11305-51, 11305-59, and 11305-59. Would the last cpt code require another modifier?

11305-59 is the correct way.

51 is not required for Medicare. Please check with the payor if they require Modifier 51
