The doctor did excision subcutaneous soft tissue all less than 3 cm. The codes I am billing are 24075 x 4 (3 on rt and 1 on lt), 25075 x 3 (2 on rt 1 on lt), 22902 x 1. How do I bill and which codes get the modifiers?

There are various ways of billing these codes depending upon the carriers' choice.
1. You can code as per number of units as you mentioned and descending order of RVUs,like:
24075 x 4 (units), 25075-51 x 3 (units), and 22902-51 (1 unit).

2. You can also use modifier 59, LT or RT with the code if appropriate and instructed by the carrier.
