I am not sure how to bill for this hospital:
5/21/15-admit
5/28/15---d/c to swing bed(do I used a discharge code)
then-
5/28/15---admit
6/2/15---d/c to swingbed (do I use a discharge code)
then---
6/2/15---admit
6/10/15--d/c to skilled nursing facility..(how do I code this?)

Well, discharged and readmit can be coded when a patient is discharged from inpatient status and is readmitted to the same facility on the same day for symptoms unrelated to the prior stay’s medical condition. Otherwise it is advisable to code subsequent visit. Please be aware that upon request of the Quality Improvement Organization (QIO), hospitals will be required to submit medical records pertaining to the readmission. Hope it helps!
