help with diagnosis

Patient presents to the ER with bleeding from dialysis shunt in left upper arm. The patient went to dialysis center and when the bandage was taken off of his left upper arm a "suture" pulled out of his shunt and blood started squirting out of shunt.

The ER physician diagnoses hemorrhage '459.0', but is this the correct diagnosis in this case since there was bleeding from the shunt?
