9x13 LLL lesion; 6x4 upper eyelid lesion
The lower lesion was measured as 9mm x 13mm and upper lesion was measured as 6mm x 4mm . The lesion was outlined with marking pen. The lesion was outlined with a #15 bard parker blade. Westcott scissors were used to excise the tissue. The #15 bard parker blade was then used to make and infra-ciliary incision in the lower lid and a supra-cilliary incision in the upper eyelid. The superficial skin was undermined with steven's scissors to create a flap. Using 5-0 fast suture, interrupted sutures were placed to close the defect. Was a tre closure done and can I use the measure of the leasion for the ATR ???? cpt code 67840

CPT code 67840 is mainly used for removal of lesion which includes more than skin (ie, involving lid margin, tarsus, and/or palpebral conjunctiva). If the detailed procedure states that the procedure was performed on the skin level with ATR, you cannot report lesion excision separately.
If the provider performs adjacent tissue transfer (ATR) along with lesion removal then, CPT code 14060 (Adjacent tissue transfer or rearrangement, eyelids, nose, ears and/or lips; defect 10 sq cm or less) with modifier E1 (Upper left, eyelid) and E2 (Lower left, eyelid) would be appropriate.
