Hardware removal humeral shaft

What anesthesia code would you recommend for removal of hardware from the humeral shaft? We usually start with the surgical CPT code and use the ASA Crosswalk to arrive at the anesthesia code. The surgical CPT is 20680, and if using the Crosswalk, the only anesthesia code that would come close to the humeral shaft would be 01630. According to the operative report - "I opened up the old incision nearly from the shoulder to the elbow and extended down through subcutaneous tissue. I got down to the biceps and went lateral to the biceps root and checked the biceps medially. I split the brachia radialis and basically came onto the plate proximally and continued staying on the plate working down distally until I got down to 1 inch or so from where I thought the radial nerve could be and then did a blunt dissection distally."
