What do I need in addition to 28285, 28270 for the following: DIAGNOSIS: Right foot fixed hammer and mallet toe with soft corn between the great toe and second toe. PROCEDURE: 1.DIP and PIP arthroplasty of the second toe, right. 2.Capsulorrhaphy of the second MTP joint with pinning of the second toe, right. 3.Dorsal osteophyte excision, first metatarsal head, right. 4.Akin procedure, proximal phalanx great toe and pinning, right. Hi,

The most appropriate CPT codes are:

PROCEDURE: 1.DIP and PIP arthroplasty of the second toe, right. CPT 28285

2.Capsulorrhaphy of the second MTP joint with pinning of the second toe, right. CPT 28270

3.Dorsal osteophyte excision, first metatarsal head, right. CPT 28288

4.Akin procedure, proximal phalanx great toe and pinning, right. CPT 28310

But, as per CCI edits, CPT 28288 is bundeled with 28270 and CPT 28310 is bundeled with 28270. If you wish to bill this scenario, a modifier is allowed to override this relationship, in both cases. As per my suggestion, I would suggest to code CPT 28285 and CPT 28270.
