The provided documentation does not show that Septoplasty was performed. So, code 30520 cannot be reported based on this information. Please make it sure with the medical record.

 

Total ethmoidectomy has been performed (anterior and posterior), so code 31259 is correct to bill.

 

Maxillary antrostomy has been performed with removal of tissue (polyp) from maxillary sinus. So code 31267 is appropriate to report.
