Patient developed granulation over punctum that had been plugged with Oasys plug. The Dr removed it with a spud. What procedure and diagnosis would be appropriated

Unfortunately, there isn't a direct procedure or diagnosis code for that scenario. For best fit, I would try either:

Procedure Code 67840 - Excision of lesion of eyelid (except chalazion) without closure or with simple direct closure.
ICD-9 375.81 – Lacrimal Passages, Granuloma

Procedure Code 68530 - Removal of foreign body or dacryolith, lacrimal passages.
ICD9 930.2 – Foreign Body in lacrimal punctum

I ended up using 67840. Thanks

You are welcome!

I just noticed that Dx is not acceptable for that procedure. I had to use 372.61 with the 375.81
