Need help with dx & procedure Codes: Pre-Op dx is foreign body, abdominal wound Post OP dx is suture granuloma subfascial involving small bowel. (site of J-tube) Procedure - Wound exploration with removal of suture and repair of small bowel enterotomy:The previous incision was reopened for a distance of about 3 cm. Bleeding is controlled with cautery. Dissection is carried down to the subfascial where a piece of Ethibond suture is encountered. This is removed. There is also a silk suture which is attached to the small bowel. This evidently was a stitch used to keep the small bowel against the abdominal wall. This is removed. A small enterotomy is inadvertently made. This is closed in 2 layers using interrupted Vicryl suture. Omental patch is then placed over this and held in place with Vicryl suture. The wound is irrigated. The abdominal wall was closed in one layer with interrupted 2-0 nylon suture. Dressing is placed. Thanks

Hi Desika,

The appropriate code to bill for granuloma suture removal is cpt 49402 and the ICD-10 code will be L76.82. Please feel to ask for any further query. Thank you.
