would it be correct to apply modifier 51 to additional 5cm of complex closure, OR just a 59 OR
Would I apply the 59 AND the 51 to the same 5 cm of additional closure length.?

If procedures are performed on the same patient, by the same physician, on the same day, during the same encounter, append modifier 51 (Multiple Procedures).
If procedures are performed on the same day, but the excisions or wound repairs involve different anatomic sites, than append modifier 59 (Distinct Procedural Service).
So you can append any one modifier from both the modifier accordingly. No need to append both modifier together.

in reply to modifier on closure, wouldn't the 51 reduce the the additional charge for the 5 cm ?

Yes,It will reduce the additional charges for the 5cm because all the material and equipment, used for additional work, that used in main procedure. There is no extra material used for additional work. So, yes charges will reduce after appending modifier 51.
