When billing 45382 and 45385 can we still use 59 modifier or should be billed as follows:
I understood we can still use 59 modifier for Medicare?

You can code this scenario with 45382-59 and 45385.
To use modifier XS, it should be mentioned in documentation that the bleeding and polyps/tumors are present at separate structure/site, but this information is missing here. If both are present at separate sites/structures, we will use XS modifier in place of 59.
