Gadolinium (Magnevist) for MRI's

Our MRI facility uses Magnevist in single use doses in vials of either 15ml or 20ml. If we only use a portion of one of the vials say 11ml of the 15ml vial how do we determine how many ml's to charge (15 or 11). Can we charge for the entire 15ml vial or only the 11ml that was used? If only the 11ml, can we somehow charge for the 4 mls that was discarded? If so, especially when billing Medicare how do we do that? Thank You!

Per the above documentation, Magnevist is a per ml code, so to bill the correct units, you need to mention the amount of drug administered in "ml" and for the discarded drug, append modifier JW along with the HCPCS code of Magnevist.

For eg: If a total of 15ml vial was used, then the appropriate billing would be, A9579- 11 units (actual drug administered) and the remaining discarded drug as A9579- 4 (units) with JW  modifier.
