Could I get some clarification, "G0461" and "G0462" will replace CPT code "88342" for Medicare claims beginning Jan 1st 2014, is this change for regular Medicare only or does it also include Medicare replacement plans such as Humana, Coventry, Medicare Complete just to name a few?

Medicare requires G0461 and G0462 instead of 88342 and +88343 as of Jan. 1. Regarding whether “Medicare replacement plans such as Humana, Coventry, Medicare Complete” require the ‘G’ codes, you’ll need to ask the specific payers.
