How is code G0325 billed if it is done in both arms and an E/M new visit is done on the same day?

G0365 is not allowed for bilateral payment. The provider performs this procedure for a patient such as an end stage renal failure patient whose condition is such that physical examination is not sufficient to select a vein suitable for creating a hemodialysis access such as a dialysis fistula, a surgically created communication between an artery and vein in an extremity for dialysis. The provider does vessel mapping by doppler ultrasound or alternative technique such as venogram to help to identify the internal structure of the blood vessels and to find a suitable site for the dialysis fistula. You can code an E&M visit but usually this procedure is followed by surgery (creation of fistula).
