Fulgaration using a hot snare for tumor destruction

Normally when I see hot snare used with fulgration, they always state the polyp was removed and fulgrated, so I always bill 43254/45385; in this case, the doctor is stating in the findings that there was a tumor in the lower third esophagus so he says he went in and did "fulgaration using a hot snare for tumor destruction"; then in the summary portion, he says "Laser therapy for tumor destruction in the lovwer third of the esophagus was successfully performed." I am conflicted if I should use CPT code 43270 since he is stating it was destroyed, or use CPT code 43254 because it says using a hot snare. Help please!

Hi Sherry, As per my understanding, since there is mention of tumor and laser therapy for its destruction, we must go with CPT 43270.
