I have a provider that saw a patient on Monday, coded a 99214, saw the same patient the next day for a follow up, coded a 99213 and 10060--is this correct coding or should the e/m for the f/u visit be a 99212?

E/M level decided on the basis of History, Physical exam and MDM (HEM) or on the basis of time. As your patient is established, CPT code series 99211-99215 can be used for second visit also- on the basis of two out of three element i.e. HEM. (2/3 rule). Use modifier 25 with E/M code if you are coding 10060.
