I think you should use an unlisted code (64999) for above scenario. What are the methods used for craniotomy? Actually there are various techniques physician can use for cranium incision such as burr hole or trephine, drill hole, puncture etc. If physician uses a burr drill or trephine to hole the cranium for biopsy, you could use 61140 (meningeal biopsy). The meningeal biopsy is considered incidental to brain biopsy. If a stereotactic technique was used, you should look for 61750 or 617151, but here no one code exact specifies the requirement.The dural repair is normally included in the craniotomy code as part of the closure and not separately reportable. Dx code for CNS inflammatory disease is 323.9.
