Forms to be filled out

If a patient is seen by a provider for a physical and later comes back and asks for a form to be filled out for WIC or sports physical, can the patient be charged for filling out the form? and if so what code?

Hi Kayme, There are two ways to bill for this scenario: Firstly, we can begin using 99212 or 99213, depending on which two of the three key components are met, OR using the time spent face to face with the patient as the determining factor. Secondly, we can use Local code (LU021), depending on the payer. If the child/adolescent has already had a comprehensive exam and the agency is just filling out the form based on what is in the clinical record from that visit, LU021 could be an appropriate code to use. Before billing it becomes very important to check with your insurance company if they bill for such codes. Please see the link provided below for further clarity.
