Follow- up colonoscopy

Could someone clear this up for me? If a Medicare patient has a hx of colon cancer or polyps and has a colonoscopy due to high risk and polyps are found we code the hx code first then the polyp. My question is: Does this patient qualify for a follow-up colonoscopy the next year with an indication for high risk since we append the PT modifier to the diagnostic procedure code showing this was done as a high risk screening? Any help with this issue would be appreciated.
