follow up codes

When using follow up codes such as v67.9 or v67.59 would the follow up code be first followed by the dx code? What if the dx has been resolved? Is there a code for follow up on labs? I have a claim that the ins has paid for the b12 shot and injection but included the office visit. Received notes and documentation supports the E/M. Are there any other options I can take? Maybe adding a modifier to the E/M

V67.59 AND V67.9 should be Primary Dx.
Follow-up on labs can be coded as V67.9
