Are follicular Neoplasm always malignant, in viewing the path report it dosen't indicate malignancy

No. The ICD-9 code for benign follicle, nabothian neoplasm is '219.0'.

I'm speaking of follicular neoplasm in reference to the thyroid?

Oh. For benign thyroid gland neoplasm, you would code '226'. For all neoplasm coding you have the choices of primary malignant, secondary malignant, ca in situ, benign, uncertain behavior and unspecified. Each with a different ICD-9 code.
