2.Right ring PIP gouty arthropathy with large tophus formation.
3.Ossific fragment, right ring radial PIP level.

PROCEDURE:
1.Right index A1 release and limited flexor tenosynovectomy.
2.Right ring ossific fragment excision, radial PIP level.
3.Excision, gouty tophus from right ring PIP joint dorsal ulnar and radial level.

I would suggest the appropriate cpt codes with undermentioned scenario

1.Right index A1 release and limited flexor tenosynovectomy 26145-F6
2.Right ring ossific fragment excision, radial PIP level 26235-F7
3.Excision, gouty tophus from right ring PIP joint dorsal ulnar and radial level 26080-51-F7

If Right ring ossific fragment excision, radial PIP level performed at the deep inside the joint level bill cpt 26145-F6 and 26080-F7; don't bill cpt code 26235-F7.
If ossiffic fragment excision was performed around the PIP then bill cpt's as 26145-F6, 26080-51-F7, 26235-F7.
