How would I code the following:
DIAGNOSIS: Left index finger dorsal soft tissue swelling compatible with hypertrophic granulation and chronic ulcerative type lesions.

PROCEDURE: Irrigation and excision of soft tissue granulation tissue and ulcerative lesions, dorsal left index finger. Measurement was 2.5 cm x 4 mm x 5 mm.
