What are the CMS or coding guidelines for when it is appropriate to code Z80.3 for a patient with a family history of breast cancer. What qualifies a patient to be considered family history? Does it have to be a 1st degree relative ie. mother, daughter, sister? Or can it be Grandmother or aunt? Are these guidelines documented anywhere?

A family medical history is a record of health information about a person and his or her close relatives. A complete record includes information from three generations of relatives, including children, brothers and sisters, parents, aunts and uncles, nieces and nephews, grandparents, and cousins.

Just so I'm clear, we can code Z80.3 on a Medicare claim if the patient has a Grandmother that had breast cancer?

Yes, you can report Z80.3 (Family history of malignant neoplasm of breast) if the patient has a Grandmother that had breast cancer.
