External neurolysis

My Dr. did a re-do laminotomy, foraminotomies L3-4 and L5-S1, with discectomy at L5-S1, He also did external neurolysis L5, S1, L4 nerve roots on the left. I have coded 63042,63044 and want to use 64714. My question is how many units can you use for 64714 if there were 3 nerve roots?? One or three units?? Or is 64714 inclusive with 63042?

CPT 63042 is for laminotomy (hemilaminectomy), with decompression of nerve root(s), including partial facetectomy, foraminotomy and/or excision of herniated intervertebral disc, re-exploration, single interspace and CPT 63044 is an add-on code for each additional interspace. CPT 64714 is for neuroplasty, any major peripheral nerve, MUE for this code is 1, so you can bill only one unit for this code. As per CCI edits CPT code 64714 is a column 2 code for 63042, so modifier is allowed in order to differentiate between the services provided. Append modifier 59 with CPT 64714.
